Union Bank

Below are the instructions to enroll your UnionBank Account with Plentina as a biller. Enrolling your Bank Account will allow you to gain access to our full suite of products on the Plentina app with payments automatically deducted hassle-free!

Before proceeding, please make sure that your bank account is UnionBank.

1. From your Plentina app, tap on “Me” on the lower right and copy your Customer ID number, you will need this written on your form.

2. Download and print the ADA form here.

3. Fill out the form with the following  details using a blue pen:

On first page:

  • Depositor/Subscriber Name (Last Name, First Name, Middle Name)
  • Email Address
  • Mobile Number
  • Depositor Account Number - Unionbank 12-Digit Payroll Account Number
  • Subscriber / Reference Number - Plentina 12-Digit Customer ID Number
  • Subscriber’s Authorized Signatory - Your Signature over your Printed Name
  • Date - Current Date
  • Leave all other fields blank

On second page:

  • Subscriber’s Authorized Signatory - Your Signature over your Printed Name
  • Date - Current Date

4. Take a photo of the 2 pages of your printed form and send via email:

  • emailto: employeebenefits@plentina.com 
  • Subject: Company_Last Name_First Name
  • Body: Preferred Mode of Communication: email/mobile/viber
  • Contact Number: personal mobile number
  • Attachment: photos of the 2 pages of the filled out UBP ADA Form using blue pen

5. Once you receive a response on your email confirming your entry, you can proceed with delivering the signed ADA form to [For Discussion: Where Ops Drop Off or can be direct to Bank HQ - WetSig Option 2c or 3?]
